Don’t forget to update your contact information with the Per Capita office before the end of the month. In order for any changes to take affect for the December payment, all updated forms need to be turned in by MONDAY, November 30th.

– W-9

– Per Capita Payment Options

– Voluntary Garnishment Agreement

– Voluntary Tax Withholding
